Anatomic variance of the coracoclavicular ligaments.

چکیده
The coracoclavicular ligaments vary widely in morphology and anatomic descriptions. Few authors have adequately described the coracoclavicular ligaments' anatomy, and a number of discrepancies exist in the anatomy literature. This study examines the complex anatomy of the coracoclavicular ligaments and their relationships to clinically important bony landmarks. The geometric dimensions of 24 coracoclavicular ligament specimens from fresh human cadaveric shoulders were examined and quantified with 13 different measurements. Particular attention was given to any inter-specimen anatomic variance. The coracoid insertions of the conoid ligaments displayed high variance, with 33% (8/24) being confluent with the lateral fibers of the superior transverse scapular ligament. A further 15% (3/24) presented an additional lateral fascicle. The distance from the lateral trapezoid ligament to the distal clavicle averaged 15.3 mm. Three distinct and previously unreported conoid ligament variants lend themselves to an anatomic classification (types I, II, and III). A safety margin of 15 mm is suggested for distal clavicle resection in incomplete acromioclavicular joint injuries to preserve the intact coracoclavicular ligament.

Changes in surgical procedures for acromioclavicular joint dislocation over the past 30 years.
Generally, surgical treatment is recommended for Rockwood type 5 traumatic acromioclavicular joint dislocations. Since 1980, the authors have performed the modified Dewar procedure, the modified Cadenat procedure, and anatomical reconstruction of the coracoclavicular ligaments for this injury. The goal of this study was to determine the ideal surgical procedure for acromioclavicular joint dislo...

INTRODUCTION Acromioclavicular (AC) joint separations are common injuries seen in athletes or laborers in which the AC capsule and coracoclavicular (CC) ligaments are both ruptured [1]. Both surgical and conservative treatments have similar outcomes [2], with both treatments having no guidelines for rehabilitation. The CC ligaments, trapezoid and conoid, have greater structural properties than ...
متن کاملManagement of acromioclavicular joint injuries.
Acromioclavicular joint injuries are among the most common shoulder girdle injuries in athletes and most commonly result from a direct force to the acromion with the arm in an adducted position. Acromioclavicular joint injuries often present with associated injuries to the glenohumeral joint, including an increased incidence of superior labrum anterior posterior (SLAP) tears that may warrant fu...
متن کاملArthroscopically Assisted Fixation of Unstable Distal Third Clavicle Fractures
INTRODUCTION Clavicle fractures are common injuries, representing 35 to 45% of fractures of the shoulder girdle. Fifteen to 25% of those involve the distal portion of the clavicle. Previously, non-operative management of midshaft clavicle fractures was based on the premise that even grossly malunited fractures had a minimal impact on functional outcomes. However, level I evidence supports the i...
متن کاملArthroscopic anatomical reconstruction of the acromioclavicular joint.
Traditional techniques for restoration of a separated acromioclavicular joint are afflicted with various complications and often lead to recurrent dislocation. Lately, anatomic as well as minimally-invasive repair techniques with major focus on restoration of the coracoclavicular ligaments have been described for acromioclavicular joint reconstruction. We present a technique for an arthroscopic...
